Home and Community Assistance : Home and Community-Based Services (HCBS) are a supportive range of services designed to help older adults and individuals with disabilities live independently in their own homes and communities, rather than in institutional care. This person-centered model includes in-home assistance with daily personal tasks like bathing and meal preparation, as well as community-based support such as adult day services and transportation. The core goal of HCBS is to empower individuals by providing the necessary care that honors their dignity and preferences, allowing them to maintain their quality of life and age in place safely and comfortably.

Community Transition : The community transition movement, when applied to the domain of homecare, represents a paradigm shift from fragmented, institutional models toward integrated, community-embedded support. It fosters local ecosystems where care is not a transactional service but a reciprocal network, empowering individuals to age or convalesce in place. This is achieved by strategically linking professional homecare providers with neighbors, volunteers, local businesses, and social services to create a fabric of practical aid, social connection, and shared responsibility. The result is a more resilient, compassionate, and cost-effective care model that enhances individual dignity, reduces isolation, and strengthens the community’s intrinsic capacity to care for its own members.

SFC

Structured Family Caregiving (SFC) is a Medicaid-funded model that allows family members to be paid a stipend for providing care to a loved one at home, supported by a professional agency for training and oversight.

Nutritional supplements

A nutritional supplement is a product designed to provide essential nutrients—such as vitamins, minerals, amino acids, herbs, or enzymes—that may be missing or insufficient in a person’s regular diet. These products, which come in forms like pills, capsules, powders, or liquids, are intended to complement daily food intake, not replace it. They serve various purposes, from filling dietary gaps and supporting overall wellness to addressing specific deficiencies or health goals, such as boosting immunity, improving bone health, or enhancing athletic performance. However, their effectiveness and safety depend on individual needs, and they are most beneficial when used under guidance, as they are not strictly regulated like pharmaceutical drugs.

Attendant Care:

“Attendance Care” is a model of support focused on the essential presence and supervision of a caregiver for an individual who cannot be safely left alone due to a physical, cognitive, or behavioral condition. Its primary purpose is to ensure safety and provide hands-on, non-medical assistance with Activities of Daily Living (ADLs) like bathing, dressing, and mobility. Unlike skilled nursing, it does not involve complex medical procedures, and it is often a defined benefit in contexts like long-term care insurance or Veterans Affairs “Aid and Attendance” pensions, where the critical factor is the necessity of having a caregiver in attendance to prevent harm and support fundamental personal needs.

Home Delivery Meals:

Home delivery meal services play a crucial role in home care by providing reliable, nutritious, and convenient food solutions that directly support an individual’s ability to age or recover in place. These services address key challenges such as difficulty with grocery shopping, meal preparation, or specific dietary needs due to health conditions, mobility limitations, or cognitive decline. By delivering balanced, often chef-prepared meals directly to the door, they ensure clients receive proper nourishment, which is fundamental for maintaining strength, managing chronic illnesses, and promoting overall health. This practical support reduces caregiver burden, enhances the client’s independence and quality of life, and significantly lowers the risk of malnutrition and food-related safety concerns in the home. Personal Care

Non Medical Transportation- UnassistedThis designation means the transportation service is strictly for mobility and conveyance, not for medical care. The client is considered “unassisted,” indicating they are able to get in and out of the vehicle independently, secure their own seatbelt, and travel without the need for a driver or attendant to provide physical support, medical monitoring, or assistance with personal needs. The driver’s role is solely to operate the vehicle. This type of service is commonly used for trips to medical appointments, dialysis centers, adult day care, or grocery stores for individuals who are ambulatory or use a walker or wheelchair but can manage their own transfers without hands-on aid. It is a key benefit in many Medicaid or insurance plans, designed to overcome the barrier of lack of transportation while distinguishing it from a more skilled “assisted” medical transport where staff are trained to handle patients with greater needs.
